The ESSENTIAL Comfort Package is a cost effective solution for your face down needs. It includes the Sleep/Portable Support for comfortable sleeping. Also, use at a table top and riding in the car.
Rental Terms & Conditions
There is a one (1) week minimum on all rentals.
For purposes of billing the rental period begins on the day of the lessee's surgery or, in cases where the equipment was shipped and received after the patient’s surgery, the day following delivery of the equipment to the patient’s shipping address. The rental period ends on the day prior to the date the equipment is received by UPS for return shipping.
Vitrectomy Recovery Solutions requires that the first week rental fee plus shipping charges be pre-paid by credit card before the equipment is shipped. If you do not have a credit card, you can pay by Debit Card, Check, Money Order or COD (cash on delivery) for the full amount before the equipment is shipped.
Additional rental weeks beyond the first week are assessed the weekly rate and are charged to the card on file at the end of each additional full week of use. Additional rental times of less than a full week are assessed the daily rate and are charged to the card on file upon return of the equipment to an Authorized UPS Shipper.
Equipment must be returned in good condition, allowing for normal wear and tear. If the equipment or any part of the equipment or any accessory included with your rental are not returned or are found to be damaged due to patient misuse or neglect, additional charges for reconditioning or replacement of the equipment or accessory may be charged. If the equipment is needed for additional periods of time, please contact our office to make arrangements.
Cancellations
Rental reservations can be canceled up to and on the day prior to the scheduled ship date at no charge. If a rental reservation is canceled after the equipment has shipped, the first weeks rental fee plus shipping will be charged. There are no refunds for rentals that have shipped.
If an order is canceled due to lessee’s surgery being rescheduled the rental charges will be credited when the lessee re-rents and the lessee will only be charged the additional shipping and handling charges.
